It is time to renew your Youth Health Forms. This form contains all of the medical/health/insurance information that we usually ask for each and every trip. The idea is to have a form for each youth that will be attending outings with the Williamson's Chapel youth group. You fill it out once at the beginning of the year and have it notarized (please make sure you sign the form in front of the notary and be prepared to show ID). Unless any of your child's health or insurance information changes you will not have fill it out again until 2012/2013 School Year. Please be aware that at times conference forms may still need to be completed (as is the case with Kaleidoscope). This form needs to be notarized and Libra Pitts is a notary, so if needed you can stop by the church office to have the form notarized.
